Use this form as a guide in recording a denial of access to a public record. Also for use in either the challenging or the denial or supporting an argument for changing the law.

1. Describe the record you requested, including its location and supposed custodian.
2. Was your request oral or in writing? Attach written request.
3. If your request was oral, quote the way you described the record as accurately as possible.
4. To whom did you make the request, name and position?
5. On what date did you make the request?
6. Did you receive a reply; if so when and was the reply in writing?
7. Attach a written reply; describe an oral reply in as much detail as you can remember.
8. What was the subject of the story for which you requested the record?
